Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. This telephonic role focuses on patient education, managing disease states, and optimizing therapy through detailed documentation and collaboration with prescribers.

The successful candidate will reconcile medications, assess interactions, and provide side effect management recommendations. The position requires a pharmacist license, 2+ years of experience, and a strong clinical background, with a bonus potential of 7.5%

Qualifications

• Current, unrestricted pharmacist license in state of residence.
• Minimum two years of practice experience as a licensed pharmacist.
• Strong clinical background in pharmacy and patient management.
• Experience with specialty medications desirable.

Responsibilities

• Provide telephonic patient consultations on specialty medications.
• Perform medication reconciliation and screen for interactions.
• Interpret prescriptions and verify details with prescribers.
• Document clinical interventions in pharmacy software per policy.
• Expedite order processing and meet production metrics.
• Support quality standards and resolve patient issues.
• Educate patients and caregivers on therapy and safety.
• Back up other pharmacists and train staff as needed.
• Comply with all applicable laws and ethical standards.
