Job summary

Provident Behavioral Health is seeking a Personal Care Assistant to deliver in-home care, ensuring safety and quality of life for clients. You will follow individualized care plans and document tasks via EVV or timesheets, while supporting daily living activities with empathy and respect.

The role requires 6 months of caregiving experience (preferred), a high school diploma or equivalent, and strong communication and safety awareness to maintain patient-centered care.

Qualifications

• High school diploma preferred; ability to read and write required.

Responsibilities

• Following individualized care plans and completing daily tasks via EVV or electronic timesheets.
• Assisting clients with personal care such as bathing, grooming, ambulation, feeding, and using assistive devices.
• Encouraging self-help and independence while maintaining safety protocols and infection control measures.
• Performing secondary tasks like light housekeeping, laundry, grocery shopping, and meal prep within client boundaries.
• Monitoring client condition, reporting changes, and notifying supervisors of safety concerns or unusual activities.
• Respecting client privacy, confidentiality, and patients' rights at all times.
• Assisting with medication self-administration (not direct administration) and ensuring a clean, safe environment.
• Reporting any abuse, neglect, or unsafe conditions promptly.
