Job summary

Health Wealth Safe seeks Personal Care Aides to provide in-home care support to clients, following individualized care plans and ensuring safety and quality standards are met.

Responsibilities include following client care plans, assisting with personal care such as bathing, grooming, ambulating, and meal preparation, meals and diet needs, and basic care tasks with transfers. Maintain a safe environment and report concerns promptly.

Qualifications

• High school diploma preferred; ability to read and write required.
• Must have at least 6 months of verified caregiving experience.
• Must be 18 years or older.

Responsibilities

• Follow client care plans and complete daily tasks via EVV or electronic timesheets.
• Assist clients with personal care such as bathing, grooming, ambulating, and meal preparation, including special diet needs.
• Provide basic care tasks like feeding, oral hygiene, and assisting with transfers using equipment like walkers or wheelchairs.
• Maintain a safe, clean environment and adhere to infection control and universal precautions.
• Perform secondary tasks such as laundry, cleaning, grocery shopping, and meal prep as needed.
• Respect client confidentiality, privacy, and rights while delivering quality care within scope of practice.
• Monitor client conditions, reporting any concerning changes or unsafe situations to supervisors.
• Assist with self-administration of medications, but not administering medications directly.
• Report suspected abuse, neglect, or privacy violations immediately.
