Job summary

Hill Country Mental Health and Developmental Disabilities Centers seeks a Service Coordinator for Hays County. The role assesses needs, advocates for clients, and develops Person Directed Plans in collaboration with service providers.

Qualifications

• Bachelor's or advanced degree from an accredited college or university is required.
• Associate degree in social, behavioral health, or related field is acceptable.
• High school diploma with two years of paid/unpaid experience with individuals with intellectual or developmental disabilities is acceptable.

Responsibilities

• Assess individuals' needs and develop and monitor Person Directed Plans.
• Provide skills training to improve daily life.
• Collaborate with providers to coordinate care and outcomes.
• Document clearly with proper grammar for valuable records.
• Schedule and conduct meetings according to guidelines.
• Maintain compliance with legal requirements and policies.
• Respond to crisis with minimal supervision.
• Meet regularly to review progress and ensure compliance.
• Protect confidentiality per HIPAA.
