Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking an AOT Care Coordinator to support individuals in the Hays County Assisted Outpatient Treatment Program. You will assess needs, develop person-centered recovery plans, and coordinate services including housing, employment, and crisis intervention.

The role requires driving within the community, flexible hours, and travel to client homes and settings.

Qualifications

• Bachelor's degree in psychology, sociology, behavioral or human services.
• Able to obtain QMHP-CS credential.
• Valid Texas driver’s license and auto insurance.
• PMAB, First Aid and CPR certifications within three months of employment.
• 3–5 years of mental health/community-based service experience.

Responsibilities

• Ensure that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS/ANSA's completed on time).
• Achieve an average score of 85, with no one score below 75 on the Person Centered Recovery Plan audits.
• Schedule and deliver services that meet performance targets relevant to the position.
• Responsible for ensuring staff kept appointment rates of at least 80%.
• Provide collaborative documentation at least 95% of the time or within 24 hours after service delivery.
• Provide hours as required by the Level of Care served.
• Provide at least 75% Person Centered Time each month.
• Deliver services using Evidence-Based Practices as recommended by HHSC.
• Develop Person Centered Recovery Planning with the individual/family served and others requested.
• Provide crisis intervention services as needed.
• Must be crisis-certified within 120 days of employment.
• Travel, home visits, and service provision in community settings.
