Job summary

Hill Country Mental Health and Developmental Disabilities Centers seeks a Service Coordinator to assess individual needs, advocate for clients, and develop Person Directed Plans. The role collaborates with service providers to support outcomes and provides skills training and crisis prevention while documenting precisely for accountability.

The position requires a bachelor’s or advanced degree (or an equivalent pathway), valid Texas driver’s license, CPR and behavioral intervention training

Qualifications

• Requires a bachelor's or advanced degree; alternative pathways include associate degree with relevant experience or a high school diploma with two years' related experience.
• Proven ability to write and document clearly and adhere to guidelines.
• Experience working with individuals with intellectual or developmental disabilities preferred.

Responsibilities

• Assess individual needs and collaborate to develop and execute life management and Person Directed Plans.
• Provide relevant skills training to improve daily living.
• Collaborate with providers to ensure effective care and positive outcomes.
• Document articulately with correct grammar for clear records.
• Schedule and conduct meetings according to state and company guidelines.
• Maintain compliance with legal requirements and policies.
• Respond to crisis interventions with appropriate supervision.
• Meet regularly with individuals to monitor progress and compliance.
• Guard confidentiality and adhere to HIPAA regulations.
