Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Service Coordinator to assess individuals' needs, advocate for them, and develop and monitor Person Directed Plans.

You will collaborate with service providers to support effective person-centered outcomes and provide skills training, crisis prevention, and maintain positive relationships with individuals, families, and providers.

Qualifications

• Bachelor's or advanced degree from an accredited college/university.
• Associate degree in social, behavioral health, or related field.
• High school diploma with two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
• Minimum 1 year of experience working with individuals with developmental disabilities.
• Maintain valid Texas state Driver's License and auto insurance.
• CPR certification and PMAB training within the first 3 months of employment.

Responsibilities

• Assess needs with individuals to develop and execute plans.
• Provide skills training to improve daily life.
• Collaborate with providers and agencies to ensure effective care.
• Document clearly with correct grammar.
• Schedule and conduct meetings per guidelines.
• Maintain compliance with laws and company policies.
• Respond to crisis interventions with minimal supervision.
• Meet regularly to monitor progress and compliance.
• Protect confidentiality per HIPAA.
