Job summary

Volunteers of America Upstate New York in Rochester, NY is seeking a Case Manager – CBP to support individuals and families experiencing homelessness in securing and maintaining housing and income through the VOA Permanent Supportive Housing Program.

The role focuses on providing comprehensive housing case management, coordinating with Rochester Housing Authority, conducting home visits, preparing service plans, and maintaining accurate records in HMIS.

Qualifications

• Associates Degree in Human Services or related field with relevant work or life experience preferred.
• Bachelor’s degree and at least one year experience preferred.
• Clean NYS Driver’s License.
• Strong organizational skills.
• Strong working knowledge of Microsoft Office software.
• Flexibility to respond to emergency client needs.
• Evenings and weekends availability as required.

Responsibilities

• Provide housing case management per VOA policies and HUD standards.
• Collaborate with Rochester Housing Authority to provide HUD required services.
• Receive and respond to referrals within 48 hours.
• Update case management notes and reimbursement trackers.
• Schedule intake and confirm eligibility with RHA.
• Conduct regular in-person sessions and monthly home visits.
• Develop individualized service plans and biannual assessments.
• Maintain HMIS records and prepare required reports.
