Job summary

VOA Mid-States is seeking a dedicated Case Manager to support individuals and families experiencing homelessness within VOA’s Permanent Supportive Housing program in the Chancery area. You will work with a team to connect clients to medical, mental health, housing, and social services using a harm-reduction, person-centered approach.

The ideal candidate has an MSW or related master’s degree with extensive field experience, plus reliable transportation and ability to travel within the service

Qualifications

• Requires a Master of Social Work (or related Master) with 7+ years in the field.
• Bachelor of Social Work or related undergrad + 10+ years of related experience.
• Alternative: 15 years of field experience with no degree.

Responsibilities

• Outreach to community partners and representation of VOA values.
• Assess clients, develop and monitor case plans, and perform follow-up for housing and services.
• Coordinate with health, mental health, and housing providers to ensure access to needed supports.
• Document client activities within 48 hours of contact and enter HMIS/EHR data within 48 hours.
