Job summary

Volunteers of America Mid-States is seeking a dedicated Case Manager IV to support residents in Permanent Supportive Housing in Louisville. The role involves applying harm-reduction and person-centered approaches to help clients access medical, mental health, housing, and social services within VOA’s Home team.

The ideal candidate has an MSW or related master’s degree with extensive experience in homelessness services, plus reliable transportation and CTI training within 90 days of hire.

Qualifications

• MSW or related Master’s degree required; 7+ years homeless services experience preferred.
• Bachelor of Social Work or related undergraduate degree with 10+ years related work experience accepted.
• Must have a valid driver’s license, personal automobile, liability insurance, and ability to travel within service area.

Responsibilities

• Outreach to community partners and represent VOA values.
• Conduct client assessments, develop and monitor case plans, and perform follow-up activities.
• Coordinate with health care, mental health, housing, and benefits service providers.
• Ensure documentation in HMIS and EHR within 48 hours of client contact.
• Assist participants in achieving independent living within one year.
