Job summary

Coalition-For-Responsible-Community-Development is seeking an ICMS Case Manager to provide coordinated case management and support services for formerly homeless individuals with special needs at our on-site housing complex. You will deliver on-site services and link clients to life skills, money management, education, health care, counseling, and job training.

You will work with tenants to develop ISPs, maintain progress through regular contacts and meetings, and coordinate with property

Qualifications

• 1-2 years of experience or related work and BA Degree.
• Experience working with youth experiencing homelessness and housing instability, and/or experience in foster care, probation, mental health systems.
• Reliable, flexible team player who works well with minimal supervision, who has a good work ethic, and an ability to set and maintain personal boundaries.

Responsibilities

• Conduct initial housing intake and needs assessment for tenants.
• Develop Individual Service Plans (ISP) with tenants.
• Coordinate linkage to services such as life skills, money management, health services, counseling, and job training.
• Conduct a minimum of two face-to-face contacts with each tenant monthly.
• Prepare and discuss progress in Individualized Stability Plans and quarterly meetings.
