Job summary

St. Joseph Center in Venice, CA, seeks an ICMS Case Manager to provide participant-centered intensive case management, build ongoing relationships with participants, and coordinate services across Care Campus and community providers to support housing stability, health, wellness, and continuity of care.

You will complete intake, assessments, enrollment, service planning, and follow-up; connect participants to housing resources, healthcare and benefits; and document in designated systems while

Qualifications

• Bachelor’s degree in social work, psychology, sociology, human services, public health, or a related field preferred, unless otherwise required by contract.
• Experience in case management, homeless services, behavioral health, housing, healthcare navigation, outreach, or related direct services preferred.
• Experience navigating housing, CES, benefits, healthcare, behavioral health, and community-service systems preferred.
• Strong participant-engagement, assessment, service-planning, advocacy, documentation, organization, and problem-solving skills.

Responsibilities

• Complete participant intake, assessment, enrollment, individualized service planning, and ongoing case-management follow-up.
• Connect participants to housing resources, healthcare, behavioral health, substance-use services, benefits, legal resources, and other supports.
• Complete CES and other required assessments, referrals, documentation, reassessments, and service coordination in designated systems.
