Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech-Language Pathologist (SLP) to join our in-house team serving residents in a SNF/long-term care setting. The role focuses on evaluating swallowing and cognitive deficits, delivering medically necessary therapy, and developing individualized treatment plans to maximize independence.

The ideal candidate holds a valid Texas SLP license and ASHA certification, with preferred SNF/LTC experience.

Qualifications

• Active/Texas license in Speech-Language Pathology required.
• Current ASHA Certification required.
• Experience in SNF/Long term care preferred.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing.
• Provide AAC systems for individuals with language disorders.
• Develop and implement individualized treatment plans to improve independence.
• Provide training to clients, family, caregivers, and other professionals.
• Coordinate with physicians and other professionals for diagnostic procedures.
