Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house rehab team serving residents with person-centered care and positive outcomes.

The SLP will evaluate swallowing and cognitive deficits, implement individualized treatment plans, and educate residents and caregivers while coordinating with physicians for diagnostic procedures. Current Texas license and ASHA certification required.

Qualifications

• SNF/long-term care experience preferred.
• Current knowledge of SLP treatment practices.
• ASHA certification and Texas license required.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans.
• Provide AAC systems and therapy assessments.
• Educate residents, caregivers and staff.
• Coordinate with physicians for swallow procedures.
• Document billing and clinical records accurately.
• Maintain confidentiality and comply with regulations.
