Job summary

Creative Solutions in Healthcare - West is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house team. The SLP will assess swallowing and cognitive function, create individualized treatment plans, and provide education to residents and caregivers in a SNF setting.

Active Texas license and ASHA certification are required. Strong debriefing skills, documentation accuracy, and collaboration with care teams are essential.

Qualifications

• SNF/long-term care experience is preferred and welcomed.
• Current knowledge of speech-language pathology treatment practices.
• Familiarity with Net Health EHR or similar documentation systems.
• Ability to work with residents with diverse personalities and needs.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individual treatment plans to improve swallowing or cognition.
• Provide AAC systems for severe communication disorders.
• Educate residents, families, and caregivers on therapy goals and progress.
• Coordinate with physicians for orders and assessments (MBSS/FEES).
• Ensure timely, accurate billing records and clinical documentation.
• Maintain confidentiality and comply with laws and standards.
