Job summary

Creative Solutions in Healthcare - South is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care.

The SLP will evaluate swallowing and cognition, develop and implement individualized treatment plans, provide AAC systems, educate residents and caregivers, and collaborate with other professionals to ensure quality care. Texas license and ASHA certification preferred; flexible schedules available.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Help residents achieve goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech-language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for individuals with severe expressive or language disorders.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Maintain accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
