Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech-Language Pathologist (SLP) to service residents in our in-house team. The SLP will evaluate swallowing and cognitive deficits, develop individualized treatment plans, and provide education to residents, families, and caregivers to improve communication and swallowing abilities.

Responsibilities include AAC implementation, collaboration with other professionals, and timely documentation.

Qualifications

• Active Texas licensure in Speech-Language Pathology.
• Current ASHA Certification.
• In good standing with licensing boards.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve cognition or swallowing.
• Provide AAC systems for individuals with language disorders.
• Educate residents, families, and caregivers about therapy goals and progress.
• Perform skilled, medically necessary assessments and document findings.
• Coordinate with other professionals for diagnostic procedures as ordered by physicians.
