Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech Language Pathologist to join our in-house team serving residents with person-centered care to achieve positive outcomes. The SLP will evaluate swallowing and cognitive function and provide skilled therapy on an as-needed basis.

Ideal candidates hold an active Texas license in Speech-Language Pathology and current ASHA certification, with experience in SNF/long-term care.

Qualifications

• Must hold active Texas SLP license.
• Current ASHA Certification required.
• In good standing with regulatory boards.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to increase independence.
• Provide AAC systems and education to residents and caregivers.
• Coordinate with physicians for diagnostic swallowing procedures (MBSS, FEES).
• Maintain accurate billing records and clinical documentation.
