Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house rehab team serving residents with person-centered care.

The role focuses on evaluating swallowing and cognitive function, planning targeted therapy, and delivering AAC solutions as needed. Requirements include Texas licensure, ASHA certification, and good standing with licensing boards; SNF/long-term care experience preferred.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology
• Current ASHA Certification
• In good standing with all regulatory agencies and licensing boards

Responsibilities

• Evaluate residents' conditions for swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to increase independence.
• Provide AAC systems for expressive and language disorders.
• Coordinate with other professionals for diagnostic swallowing procedures.
• Provide training to clients, families, and caregivers.
