Job summary

Live Well Home Care is seeking a Per Diem/PRN Registered Nurse (RN) to coordinate Start of Care, re-certifications, discharges, and post-discharge care in a home care setting. You will provide direct patient care, administer medications, monitor vitals, and collaborate with physicians to implement care plans.

Ideal candidates have an active RN license, minimum 2 years in home care, strong communication, and knowledge of relevant policies and regulations.

Qualifications

• Active RN license required.
• Minimum 2 years of home care experience.
• Effective written and verbal communication skills.
• Experience with policy, rules, and regulations.
• Clean background check and driving record.

Responsibilities

• Coordinate Start of Care, Re-certification, Discharge, and Resumption of care.
• Provide direct patient care and monitor vital signs.
• Collaborate with physicians to develop and implement care plans.
• Educate patients and families about medical conditions and prevention.
• Assist with discharge planning and coordinate post-discharge care.
• Provide specialized care for patients with dementia or cognitive impairments.
