Job summary

Live Well Home Care is seeking a Per Diem/PRN Registered Nurse (RN) to coordinate Start of Care, re-certifications, discharges, and post-discharge care in a home care setting. Per Diem/PRN Registered Nurse (RN)

Duties:

• Responsible for coordination of Start of Care, Re-certification, Discharge, and Resumption of care
• Provide direct patient care, including administering medications, performing diagnostic tests, and monitoring patient vital signs.
• Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
• Educate patients and their families about medical conditions, treatment options, and preventive care measures.

• Provide specialized care for patients with dementia or other cognitive impairments.

Qualifications:
