UnitedHealth Group is seeking a Per Diem Registered Nurse to provide and direct nursing care to patients in their homes, coordinating care with physicians and the care team to maintain continuity. Responsibilities include assessments, eligibility verification, and developing individualized care plans.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license with access to a vehicle or reliable public transportation.

Responsabilités

• Provide high-quality clinical nursing care within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by policy.
• Develop and revise individualized plans of care with community providers.
• Ensure plan of care frequency and duration meet patient needs with physician approval.
