Job summary

Mercy is seeking a Registered Nurse for a per diem home health role. The position involves providing day-to-day patient care in patients' homes, managing admissions, discharges, and post-hospital assessments.

CPR is required biannually, and candidates must meet licensure and experience prerequisites. Candidates should have three years of nursing experience with at least one year in hospital settings; home health experience is preferred.

Qualifications

• Graduation from accredited school of nursing.
• RN license in applicable state of practice.
• Three years nursing experience with minimum one year in a hospital setting, then preferably in home care or the equivalent.
• CPR biannually.
• Must be able to lift 50#, transfer patients up to 100# with/without assistive devices.
• Ability to work in varied conditions and learn CMS Home Care rules.

Responsibilities

• Provide day-to-day patient care in the home setting as assigned.
• Perform admissions, discharge planning, and post-hospitalization assessments.
• Provide patient teaching, IV therapy administration, wound care, and dressing changes as required.
• Document appropriately in Epic and adhere to Mercy Service Standards.
