Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one, in-home skilled nursing, administer medications, wound care, and assessments. You will develop individualized care plans with physicians, monitor patient progress, educate families, and coordinate with the care team.

A valid RN license and CPR are required. This role requires collaboration with physicians and clinicians, driving to patient homes, and maintaining detailed patient records to support recovery

Qualifications

• Current, unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients’ conditions and report changes to physicians and Clinical Manager.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee the work of Home Health Aides and ensure documentation is completed properly.
