Job description

  • Support management in an administrative role to ensure a high-quality service environment
  • Serve as shift lead staff and ensure supports are delivered as designed
  • Provide individual and group support to people served in their home
  • Teach skills that enhance independence and self-advocacy and support individual choices
  • Maintain a safe, secure, clean, and well-maintained residential environment
  • Help individuals integrate into their communities and develop and maintain friendships
  • Assist individuals in accessing community-based supports and resources
  • Plan recreational and social activities promoting inclusion and emotional well-being
  • Support access to assistive technology and adaptive equipment
  • Support work, volunteer, and community engagement opportunities
  • Provide administrative support to the Program Director, including scheduling, training new staff, and overseeing clinical paperwork
  • Manage shift operations and clinical programming as designated lead staff
  • Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
  • Train and support individuals in rehabilitative and behavioral interventions, safety, appointments, money management, ADLs, shopping, meal planning, cooking, chores, and generic resources
  • Perform hands-on tasks and provide transportation as needed
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, and communication logs
  • Procure, store, administer, document, and dispose of medication in accordance with MAP requirements and policies
  • Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
  • Participate in mandated training and staff meetings and assist with new-staff orientation
  • Maintain required staff-to-individual ratios and assist with shift coverage
  • Serve as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Complete housekeeping, snow shoveling, trash removal, paperwork, logs, and other related duties
Requirements
  • One to two years of experience/training in a related function
  • High school diploma or equivalent required; Bachelor’s degree preferred
  • In some cases, experience may be substituted for academic training
  • Driving is required using either a Vinfen van or personal vehicle
  • Valid driver’s license required at employment; must be at least 21 years old, have maintained a valid US driver’s license for at least six months, and pass a driver’s screening background check
  • Personal vehicle users must maintain adequate insurance and a safe driving record
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and assist clients with daily living tasks
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to communicate effectively orally and in writing
  • Ability to work independently and as part of an interdisciplinary team
  • Knowledge of administrative practices, personal computer applications, advocacy, empowerment, self-advocacy, family partnerships, social inclusion, PBS treatment programs, participatory planning, physical personal management, household management, and crisis intervention techniques
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Positive Behavioral Supports Tier II required prior to hire
  • NET required
  • Ability to ensure compliance with regulatory policies and standards
  • Sensitivity to cultural, religious, racial, disability, and gender issues
