Job summary

Mederi Caretenders, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to support home-based care. The role emphasizes treatment implementation, patient education, and collaboration with supervising therapists to ensure quality outcomes.

Responsibilities include facilitating active/passive exercises, gait and mobility training, and coordinating with the health care team. Weekend availability is required, and CPR certification is preferred or obtainable within 90 days.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state laws governing physical therapy in home care as defined by supervising PT.
• Instruct and aid patients in active and passive exercise, gait, ADL, and mobility training.
• Observe and report patient responses to treatment to the supervising PT and care team.
• Educate patients, families, and caregivers on therapy techniques within the PTA scope.
• Function in diverse home settings and respond appropriately to patient needs.
