Qualifications

• Minnesota Physical Therapist license is required.
• Graduation from an accredited Physical Therapy program is required.
• Strong clinical judgment and patient-centered communication skills are required.
• Ability to work independently in a home health setting is required.
• Valid driver’s license and reliable transportation are required.
• Previous home health experience is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide in-home therapy focused on strength, balance, mobility, and functional independence.
• Educate patients and caregivers on exercises, safety, and continued progress.
• Collaborate with the interdisciplinary care team and maintain timely clinical documentation.
