Job summary

BrightStar Care Maple Grove is seeking a Per Diem Physical Therapist to serve the Monticello/Elk River area. You’ll travel to clients’ homes within our territory, work with VA and national accounts, and enjoy a flexible part-time schedule with weekly pay.

The role requires 1 year of PT experience, Minnesota licensure, CPR certification, and a valid driver’s license. Per diem visits pay $70 each and $100 per evaluation, with benefits including 401(k) matching, PTO, and ongoing training.

Qualifications

• 1 year of experience as Physical Therapist required.
• Current CPR Certification required.
• Current PT license for Minnesota required.
• Valid driver's license required.

Responsibilities

• Assess and evaluate patients, develop the plan of care and treatment.
• Perform visits based on the treatment plan and update progress.
