Job summary

JAG Physical Therapy is seeking a compassionate Physical Therapist specializing in orthopedic home rehabilitation for patients recovering from hip/knee replacements and other orthopedic conditions. The position is per diem in Staten Island, delivering care in patients\' homes with a plan to transition to outpatient settings for continuity of care.

The role emphasizes home-based evaluation and treatment, documentation, and collaboration with the care team to restore mobility and function.

Qualifications

• Licensed Physical Therapist in the state practicing in NY/NJ.
• Completed state CEU requirements (NY, NJ).
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision‑making skills.
• Excellent work ethic and dedication to patient success.
• Basic computer skills.

Responsibilities

• Educate patient on safe bed mobility, transfers and ambulation.
• Assess and discharge patients to home following evaluation.
• Complete daily notes and progress reports.
• Prepare evaluation with patient history and diagnoses.
• Develop and adjust treatment plans with interdisciplinary team.
• Provide education/training to patient and support system.
• Perform functional training including ADLs/IADLs and rehab equipment prescription.

Bachelor's or Master’s Degree
