Job summary

LiveWell Homecare Agency, LLC seeks a licensed physical therapist to provide orthopedic home physical therapy for patients recovering from total hip and knee replacements and other orthopedic conditions in Jersey City.

You will assess patients, develop and implement personalized treatment plans, educate on safe mobility, coordinate with interdisciplinary teams, and document care in EMR while supporting safe discharge readiness.

Qualifications

• Bachelor’s or Master’s degree in physical therapy.
• Active state licensure and CPR certification.
• Experience in orthopedic rehabilitation, esp. joint replacements.

Responsibilities

• Evaluate patients and develop personalized treatment plans.
