Job summary

JAG Physical Therapy seeks an Orthopedic At-Home Physical Therapist for per diem assignments in New Jersey. You will evaluate and treat patients in their homes, focusing on total hip/knee rehab and improving mobility, strength and function.

You will transition patients to JAG outpatient locations for continuity of care, collaborate with an interdisciplinary team, and document care using Prompt EMR while maintaining high-quality patient education and safety.

Qualifications

• Licensed PT with NJ license and CEU requirements.
• CPR certification is required.
• Experience with EMR software is preferred.

Responsibilities

• Educate patients on safe bed mobility, transfers and ambulation.
• Assist with transfer, ambulation and stair negotiation for safe home discharge.
• Document initial evaluations, progress notes, and discharge plans in the EMR.
