Job summary

JAG Physical Therapy is seeking a licensed Physical Therapist to join our home health team in Ocean County/Toms River, NJ on a per diem, part-time basis. The role focuses on orthopedic rehabilitation in patients' homes, with potential transition to outpatient visits for continuity of care.

You will evaluate and treat patients, document progress using Prompt EMR, and collaborate with a multidisciplinary team to achieve functional goals and safe discharges home.

Qualifications

• Requires Bachelor's or Master's degree; Doctorate preferred.
• Licensed Physical Therapist in NJ.
• Completed state CEU requirements (NJ).
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision-making skills.
• Team player with ability to work independently.
• Maintains a positive, professional demeanor.

Responsibilities

• Educate patient on safe bed mobility, transfers and ambulation.
• Assist with transfers, ambulation and stair negotiation for safe discharge.
• Collect and record patient data per guidelines and payers.
• Complete daily notes and progress reports; communicate with team.
• Prepare evaluations; obtain patient history and diagnoses knowledge.
• Select and administer tests and measures.
• Formulate diagnosis, goals and plan of care from evaluations.
• Provide education/training to patient and support system.
• Deliver safe, effective interventions and document with EMR.
