Job summary

JAG Physical Therapy is seeking a skilled Orthopedic At-Home Physical Therapist for per diem coverage in Middlesex/Mercer County, New Jersey. The role focuses on rehabilitation after total hip and knee replacements and other orthopedic conditions, delivering care in patients' homes to restore mobility and confidence.

You will evaluate and treat patients during the plan of care, with an aim to transition to JAG outpatient locations for continuity of care.

Qualifications

• Bachelor’s or Master’s Degree required, Doctorate degree preferred
• Licensed Physical Therapist in the state practicing in
• Completed state regulated CEU requirement in appropriate state (NY, NJ)
• Valid CPR License
• Experience/knowledge with an EMR software is preferred
• Excellent oral, written communication
• Strong clinical decision-making skills
• Excellent work ethic and dedication to patient success
• Basic computer skills

Responsibilities

• Evaluate and treat patients in the home setting for the duration of their plan of care
• Work toward transitioning care to JAG outpatient orthopedic locations for continuity of care
• Provide orthopedic rehabilitation focused on recovery after hip/knee replacement and other conditions
• Deliver compassionate, patient-centered care in patients' homes
