Job summary

JAG Physical Therapy in Morris County, NJ is seeking a Per Diem Orthopedic Home Physical Therapist to support patients recovering from total hip and knee replacements and other orthopedic conditions in their own homes. The role focuses on mobility, strength, function, and safe community re-engagement.

You will evaluate and treat patients for the plan of care at home, with potential transition to JAG outpatient locations for continuity of care.

Qualifications

• Licensed Physical Therapist in NJ.
• Bachelor's or Master's degree; Doctorate preferred.
• CPR license and EMR experience preferred.
• Excellent oral and written communication; strong clinical judgment.
• Ability to work independently in patients' homes and as a team.

Responsibilities

• Educate patients on safe bed mobility, transfers, and ambulation with post-op precautions.
• Transfer and ambulate patients; evaluate discharge readiness to home.
• Document care data per guidelines, including evaluations, daily notes, and progress.
• Complete daily billing and maintain productivity per department policy.
• Develop and adjust plans of care and goals with interdisciplinary team.
• Provide ADL/IADL training and home rehab equipment instruction.
