Job summary

Qualifications

• Graduate of an ACOTE-accredited OT program.
• Current license to practice OT in the assigned state.
• NBCOT entry-level certification eligibility.
• Current CPR certification.
• 1+ years of related OT experience; approval required if less than 1 year.

Responsibilities

• All aspects of OT service delivery under physician-approved plan of care.
• Ensure plan of care addresses identified problems; document rationale if not addressed.
• Ensure safety and effectiveness of OT service delivery and complete documentation timely.
• Supervise home health aide when RN is not active in the case.
• Other duties as assigned.
