Qualifications

• Active un-encumbered NP license to practice
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine
• CPR certification current

Responsibilities

• Conduct comprehensive member assessments in home or telehealth settings
• Document encounter findings and care recommendations
• Identify gaps in care and coordinate with PCPs and health plans
• Ensure HIPAA compliance and proper documentation
