Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner to provide per diem home and telehealth assessments across Washington County, MS. You will spend a full hour with each member to capture their needs and craft personalized care plans to keep them in the home and out of the hospital.

Candidates must hold ANCC or AANP certification and an active nursing license, with current CPR. This role offers pay of $2,400–$10,000 per month, in-home visits at $120–$140 and telehealth at $85 per

Qualifications

• Active NP license in good standing.
• ANCC or AANP NP certification (Family/Adult/Gerontology/Emergency).
• Ability to perform comprehensive in-home assessments.
• Experience with CMS/HEDIS guidelines is a plus.

Responsibilities

• Conduct thorough, documented member assessments and develop care plans.
• Document and submit encounter notes timely.
• Coordinate with PCPs and health plans.
• Maintain HIPAA compliance and confidentiality.
• Provide care recommendations to improve chronic conditions.
