Job summary

ComplexCare Solutions (CCS) seeks a Nurse Practitioner for per diem coverage across Douglas County, NE. The role emphasizes in-home and telehealth assessments to evaluate health status, gaps in care, and risks to help keep members safe at home and out of the hospital.

Applicants must hold ANCC or AANP certification, possess an active nursing license, and be CPR certified. Knowledge of CMS/NCQA guidelines is required, with proficiency in EHRs and HIPAA compliance.

Qualifications

• ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist in Family, Adult, Gerontology or Emergency Medicine.
• Active unencumbered nursing license.
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
• Familiarity with ICD-10 and CPT-4 coding practices.
• Ability to work autonomously in a remote home-based setting.

Responsibilities

• Conduct comprehensive in-home assessments and telehealth visits.
• Document encounter findings and coding in the medical record.
• Communicate assessment results and care recommendations to PCPs and health plans.
• Identify and address gaps in care and chronic disease management.
• Adhere to HIPAA and company policies; ensure timely documentation.
• Participate in 25 availability slots per month (~37 hours).
• Maintain patient confidentiality and provide high-quality customer service.
• Perform Point of Care testing when required and report results.
