Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Phoenix, AZ area. You will provide direct home infusion nursing care, educate patients and care partners, and collaborate with the care team to ensure safe, compliant treatment in the home setting.

The role requires on-call rotation, strong infusion experience, and the ability to drive to patient homes. Mileage reimbursement is provided, with paid drive time and competitive wages.

Qualifications

• Active RN license in good standing
• BSN from an accredited institution preferred
• Advanced certification (CRNI, ONS, IgRN) preferred but not required
• On-call rotation and after-hours support experience requested
• Minimum 3 years nursing experience; infusion background preferred

Responsibilities

• Provide direct patient care and initiate/maintain communications with care team
• Educate patient/care partner for safe home infusion therapy
• Assess home environment and identify safety interventions
• Develop and evaluate plan of care with team members
• Coordinate patient care via approved company platform and manage scheduling
• Participate in nurse on-call rotation to ensure continuous nursing support
• Document all patient encounters and care activities per policy
• Maintain knowledge of Soleo therapies, protocols and standards
• Ensure timely POTs and clinical documents
• Collaborate with departments to complete patient documents and consents
