Job summary

Soleo Health is seeking a Per-Diem Infusion Nurse to support infusion patients in Case Grande and Maricopa, AZ and the immediate surrounding area. The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy in the home or alternate site setting.

The schedule is Per Diem, 20-25 hours per week with a flexible plan including days, evenings, and occasional weekends.

Qualifications

• Current RN license by the state’s Board of Nursing, in good standing.
• BSN preferred, ADN acceptable depending on market conditions.
• Advanced certification in infusion or related areas preferred but not required.

Responsibilities

• Provide direct patient care for assigned patients, initiate and maintain communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Evaluate the home environment and identify safety interventions.
• Develop and evaluate the plan of care with other team members.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care via approved platform.
• Participate in the nurse on-call rotation.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POT and required clinical documents.
• Coordinate completion and return of required patient documents to branch office.
