Soleo Health in Arizona is seeking an experienced Infusion Nurse to provide direct patient care in home or alternate sites. You will assess, plan, and educate patients receiving infusion therapy, coordinating with the care team.

The role requires an active RN license in AZ, 3+ years of nursing with infusion background, on-call rotation, and willingness to travel up to 1 hour. Mileage reimbursement is provided.

Qualifications

• Current RN license by the applicable state’s Board of Nursing, in good standing.
• RN degree, BSN preferred from an accredited institution.
• Advanced certification in specialty areas (CRNI, ONS, IgRN) preferred but not required.
• Participation in an on-call rotation, nights and weekends included.
• Administrative Oversight: overseeing workflows, policy compliance, and resource management.
• Emergency Availability: respond to emergencies during non-standard hours as required.

Responsabilités

• Provide direct patient care for all assigned patients and coordinate with the care team.
• Educate patient and care partner to safely administer home infusion therapy.
• Assess home environment and identify safety interventions.
• Develop and evaluate the plan of care with the team.
• Manage schedule and communicate changes via approved platform.
• Coordinate patient care through approved systems.
• Participate in nurse on-call rotation for continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely submission of POTs and clinical documents.
• Work with departments to complete and return required patient documents.
