Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support infusion patients in Scottsdale, AZ and the surrounding area. The role includes direct patient care, education, and coordination with the care team in home or alternate sites.

Must be able to participate in an on-call rotation and drive to patient homes as needed. Ideal candidates hold an RN license with BSN and have infusion experience; ICU or Cardiac care background is a plus.

Qualifications

• Active RN license in good standing in the applicable state.
• Minimum 3 years nursing experience; infusion therapy preferred.
• Experience in ICU or Cardiac Care preferred.
• Willingness to participate in on-call rotation including nights/weekends.
• Strong knowledge of Microsoft Outlook and Office Suite.

Responsibilities

• Provide direct care for infusion patients including initiation and maintenance of all communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy and provide additional instructions as needed.
• Evaluate the home environment and implement safety interventions for prescribed care.
• Develop and evaluate plan of care with the team and assess therapy response.
• Coordinate patient care via approved company platform and manage scheduling changes.
