Qualifications

• MSW or BSW degree accredited by the CSWE.
• Must hold licensure/qualification to practice in state.
• CPR certification required by state rules.

• Previous home care/hospice experience preferred.

Responsibilities

• Assess psychosocial status of patients and families in hospice context and communicate findings to the care team.
• Perform social evaluations, address caregiver needs, and provide crisis intervention when needed.
• Educate interdisciplinary team on cultural needs and family dynamics.
• Maintain patient social work records and assist in advanced directives planning.
• Serve as liaison between patients, families, and community agencies.
• Participate in plan of care development with patient and family.
