Qualifications

Job Details — Job Location: Scarborough Main Office - Scarborough, ME 04074. Position Type: Per Diem. Category: Hospice. Department: Hospice/Home Health. Service Area: Lewiston, Maine. Work Setting: Community Care Facilities and Homes.

Role: Medical Social Worker – LCSW.

What You’ll Do:

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
• Per Diem LCSW to support Palliative Care, Hospice, and Home Health.
