Job summary

Pathways Healthcare is seeking a licensed social worker (LICSW) to join our in-home hospice team on a per diem basis. You will conduct psychosocial assessments, develop care plans, and coordinate with a multidisciplinary team to support patients and families in the Quincy, Westwood, Brockton, Scituate and Attleboro area from our Westwood, MA service network.

Qualifications

• LICSW required and CPR certification (BLS) current.
• Experience in Hospice care preferred (1+ year).
• Ability to work with multidisciplinary teams and communicate effectively.

Responsibilities

• Conduct psychosocial assessments and communicate findings to the RN.
• Develop intervention plans based on assessments.
• Maintain accurate clinical records for referred patients.
• Provide information and referrals to patients, families, and caregivers.
• Act as liaison between patients, families, caregivers and community agencies.
• Collaborate with other healthcare professionals to support patient care.
• Engage with public/private agencies to access resources for patients.
• Contribute to comprehensive care plans and case conferences as required.
• Assist physicians and team in understanding social and emotional factors affecting health.
• Contribute to discharge planning and smooth transitions.
