Job summary

Elite Hospice, part of the LHC Group within the Optum family, seeks an LMSW for Hospice Per Diem to provide medical social services under physician direction and an interdisciplinary team. This role supports patients and families and coordinates bereavement planning as required.

Key duties include assessing social factors, contributing to an integrated Plan of Care, guiding staff on community resources, and serving as Bereavement Coordinator when needed.

Qualifications

• Master’s degree from CSWE-accredited program.
• Minimum 1 year of social work in health care.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Assist patients and families in understanding important social and emotional factors affecting health.
• Contribute to Plan of Care with interprofessional team daily.
• Instruct care team on community resources available to patients.
• Serve as Bereavement Coordinator and plan bereavement services as needed.
