Qualifications

• MSW or BSW degree from a CSWE-accredited program.
• Current CPR certification as required by state regulations.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Assess the psychosocial status of patients and families in relation to terminal illness and coordinate with the interdisciplinary team.
• Provide crisis intervention and counseling to patients, families, and caregivers.
• Educate teams about cultural needs, family dynamics, and communication patterns.
• Manage clinical records for social work referrals and plan of care.
• Assist in advanced directives education and information/referral services.
• Serve as liaison to community agencies and support systems.
