Constellation Health Services is seeking a Licensed Medical Social Worker to cover our Hospice territory in Rockland, ME and surrounding area. This 32-hour-per-week role supports our hospice community with patient-family psychosocial care, care planning, and coordination with the interdiscliplinary team.

Responsibilities include psychosocial assessments, crisis intervention, advanced directive education, and liaison with community resources.

Qualificações

• MSW accredited by CSWE or BSW accredited by CSWE.
• Current state licensure to practice social work.
• CPR certification current.
• Licensed driver with insured vehicle.

Responsabilidades

• Assesses psychosocial status of patients and families related to terminal illness.
• Performs social evaluations including family dynamics and caregiver abilities.
• Educates interdisciplinary team on cultural needs and communication patterns.
• Maintains clinical records and assists in care planning.
• Provides information and referral services to patients and families.
• Acts as liaison between patients, families and community agencies.
