Broad Reach Hospice in Chatham, MA seeks a compassionate Hospice Social Worker for per diem coverage to join our interdisciplinary team. You will perform psychosocial assessments, develop care plans, provide counseling, and assist with discharge planning in both in-patient and home care settings.

Applicants must hold MA licensure (LSW/LCSW/LICSW) and have hospice social work experience or strong mental health/counseling background.

Qualifikationen

• Bachelor's or Master's Degree in Social Work, Psychology or Counseling is required.
• Massachusetts State Licensure at LSW, LCSW or LICSW is required.
• Hospice Social Work experience is preferred.
• Strong background in mental health/counseling and experience in health services delivery.
• Ability to conduct assessment of patient/family psychosocial history.

Aufgaben

• Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of the plan of care.
• Conducts ongoing reassessments of patient/family needs and counseling as required.
• Provides short term crisis intervention and individual or family counseling when indicated.
• Observes, evaluates, and brings to the team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family.
• Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and referrals.
• Documents comprehensive psychosocial information clearly and concisely in a timely manner.
• Active participant in biweekly IDT Meetings.
• Participates in clinical supervision as required by State law.
