Job summary

Chapters Health System in Florida is seeking a Per Diem RN to provide compassionate hospice and palliative care. You will assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group (IDG), and deliver care focused on comfort and quality of life for patients in our Hospice House and field locations.

Requirements include an active RN license in Florida, BLS certification, and at least 1 year of hospice or hospital experience.

Qualifications

• Current RN license in Florida.
• Active BLS for healthcare professionals.
• Minimum 1 year hospice or hospital experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provides direct care to patients and families as part of IDG.
• Coordinates Plan of Care reflecting evolving patient needs.
• Educates patients and families about disease, care, and safety.
• Communicates changes to IDG and physicians; documents thoroughly.
• Performs other duties as assigned.
